We require paperwork for every appointment, these forms help update our team on any changes in your environment since your last visit. If you are new to our practice, please make sure you are signing in on the portal versus pre-registering, pre-registering is not completing the paperwork. If you do not have a username and password for the patient portal, please contact our office at 478-333-3058. Doing the paperwork prior to your appointment helps streamline the check-in process for your upcoming visit. You can also send us messages for refills or appointment requests through the portal.
